Mission Statement:

in a safe, therapeutic, and least restrictive environment, and to establish
or return the client to optimal stabilization or functioning. Our goal
is to ensure that when care is clinically and medically necessary, it
will be provided in the most clinically and cost-effective manner possible,
and in the most appropriate, least restrictive treatment setting.

Case Management and Intensive Case Management Program
Case Management: Case Management services are designed to provide the essential linking, monitoring and advocacy to clients of the Saratoga County mental health community for the purpose of ensuring continuity of care and services. Case managers play a vital role for the client from the time of referral through discharge and aftercare planning. Case managers assist clients in the application process for appropriate services and may provide transportation when necessary.
Intensive Case Management: The Intensive Case management (ICM) program is specifically designed to service the more seriously and persistently mentally ill individuals whose treatment needs have not been adequately met by traditional mental health programs. Recipients of ICM services are adults at high risk, heavy users of emergency psychiatric services, extended care patients in psychiatric hospitals, the homeless mentally ill, and seriously mentally ill children and adolescents. Intensive case managers attempt to engage clients who previously have chosen not to use existing mental health services by providing creative options that might be more responsive to individual needs, preferences and unique circumstances.
